3rd Grade Kick Off and Return Formations

Are there any rules stating the number of players allowed on the front lines or any rules on formations for kick off and kick off return.  Other than the weight restrictions?

No you can put everyone on the frontline of kickoff return if you want.   
Kickoff team must be spread out evenly across the field and not on or behind the 35 yard line.

So is kickoff minimum 4 on one side or it has to be evenly

Depends on the placement of the tee.  If the tee is in the middle of the field then you would have to have 5 players on each side of the kicker.  If you slide it over to a hash then you would probably end up with 3 on the short side and the rest on the other side, again it is spread evenly across the field.
